Information we collect on this website

When you submit a referral or contact form, we collect the information you choose to provide — your name, email address, phone number, role, and any details you share in your message. We use this information solely to respond to your inquiry and, when applicable, to begin the referral process.

We do not use third-party advertising trackers. The site uses minimal first-party measurement to understand which pages are helpful. We do not sell, rent, or trade personal information.

Email and form submissions

Form submissions are routed through a transactional email provider (Resend) so we can receive them at our staffed inbox. Resend processes the message in transit and does not retain the body of your message beyond standard delivery logs.

Cookies

We do not set any non-essential cookies on this site. Any cookies present are strictly necessary for the site to function (for example, remembering form state during a single session).

Confidentiality of client information

Information collected during the delivery of Independent Living Services — including assessments, Individual Service Plans, progress notes, and any other client records — is treated as confidential and handled in accordance with HIPAA, the California Information Practices Act, the Lanterman Developmental Disabilities Services Act, and California Title 17 regulations. Records are stored securely and are accessible only to authorized staff and required oversight bodies.
